Rehabilitation Services
Rehabilitation services are provided on a short-term basis. The team of skilled physical, occupational and speech-language therapists outline a program specifically developed for each resident. See click here.
24-Hour Staffing
We provide 24-hour-a-day physician directed nursing care. Our professional nursing staff of registered nurses and licensed practical nurses is supported by our certified nursing assistants to provide personal care, medication administration, physician assessments and individualized care plans.
Medications
Medication needs of residents are directed through physicians’ orders that support the care plan and each resident’s health assessment. We have contact with physicians’ offices to address medication issues, changes in health status and emergencies.
Health Assessments
Health Assessments are important for measuring appropriate care and quality of life. Assessments are completed while taking into consideration the health status and physician orders for the person. Meetings with the resident and family are planned to enhance communication about health conditions and other needs.
Personal Care
Personal care is provided to help each resident look and feel their best. This includes dressing, bathing, toileting, getting in and out of bed and walking. We also have a full service hair salon.
Food
Meals and snacks are served that take into consideration resident likes, dislikes and family suggestions and doctor’s orders.
We invite family and friends for meals, especially on holidays. Private dining is available for large or small groups when scheduled in advance.
Transportation
Transportation in our wheelchair accessible van is available for scheduled physician appointments as well as outings arranged by our Activities Department.
Housekeeping/Laundry
Housekeeping and laundry services are provided at no additional charge.
Family Support
Social services are available to assist with adjustments that can often go with change in health status or disability. Supporting and advising the resident or family on the next steps can ease transitions in care and services.
Case Management services are provided for all rehabilitation patients. The Case Manager meets with the resident and family to coordinate care prior to admission through discharge. We have found this approach promotes the best outcome for the person.
